Cathy Gates, PT Practice Manager

Cathy has been a partner with Foothills since its inception in 1992. She graduated with a degree in Physical Therapy from the University of Colorado in 1976. Married to a Colorado native, she and her family have lived in Fort Collins since 1984. She has a first hand knowledge of sports injuries having had three children who played multiple sports at the High School level including a College soccer player. Cathy is now the Office Manager of both clinics, and she stays involved in all aspects of the practice. She is also active in the Colorado Chapter of the American Physical Therapy Association (APTA) and has been in charge of the student-mentoring program Regis and CU through her involvement with the private practice section of the APTA. Cathy enjoys hiking, biking, snowshoeing, travelling and volunteering with Project Self-Sufficiency. |
Jerome Kulm, PT (Loveland)

Jerome is one of the original owners of Foothills Orthopedic & Sport Therapy, P.C. He is a 1976 graduate of Loma Linda University and has worked in the Loveland/Fort Collins area for over 30 years. He has a strong clinical background with all orthopedic injuries and problems. His emphasis is in upper extremity and lower extremity injuries. Jerome is a member of the American Physical Therapy Association (APTA), including the Colorado and Local chapters and has been active as an officer for the Northeast District for many years. In 2005, he was awarded the Bob Doctor Service award for his years of dedication to his profession. He has been very active with the Sports Medicine programs of the local high schools. He enjoys golfing, hiking, motorcycling and trips with his family. |
Jeff Ray, PT (Ray PT/Fort Collins)

Jeff Ray has been working as a Physical Therapist in Fort Collins for over 25 years, graduating from the University of Colorado at Denver in 1980 with a B.A. in Biology and then in 1981 from the University of Colorado Health Sciences Center with a B.S. in Physical Therapy. He has worked with Poudre Valley Hospital, the Fort Collins Orthopaedic Associates (as Chief PT), and assisted with the transition of the Fort Collins Orthopaedic Associates into the Orthopaedic Center of the Rockies where he helped design the PT department. Jeff then joined Associates in Physical Therapy as a partner where he remained for 9 years. In April 2001, Jeff founded his own practice, Ray Physical Therapy, P.C. which merged with Foothills Orthopedic & Sport Therapy, P.C. in October 2008.
Jeff is a member of the American Physical Therapy Association (APTA). He has served as Treasurer for the Colorado Chapter of APTA, and has sat on the Judiciary/Ethics Committee. Jeff has a strong clinical background with orthopedic and sport injuries. His emphasis is in extremity and spine (neck & back) injuries. Jeff also does medical bicycle fitting.
An avid cyclist, Jeff rides his bike to work everyday in rain, wind, snow or shine. He presently races for the Vitamin Cottage/Natural Grocer's team. He also enjoys bicycle touring with his younger brother. Jeff's wife Kristy is a Red Cross Disaster instructor and FEMA reservist. Their daughter Erin recently graduated from CSU and plans to go to nursing school. |
Robert Trout, PT (Ray PT/Fort Collins)

Robert is a 1979 graduate of the Physical Therapy program at the University of Utah, and has practiced in the Fort Collins area since 1987. He began his career as the Assistant Director for the Swedish Medical Center PT department in Englewood, Colorado before entering private practice in Northern Colorado in 1982. He joined Foothills Orthopedic & Sport Therapy, P.C. in 1993 as a partner and helped establish its second location in Fort Collins in addition to the original Loveland office. Robert is a 30 year member of the American Physical Therapy Association (APTA) and a strong supporter of Private Practice Physical Therapy. His clinical background is primarily in orthopedics with a specialty in the area of neck injuries and temporomandibular joint (TMJ) dysfunction. His teamwork with dentists and physicians has provided solutions for many patients with TMJ disorders, myofascial pain and headaches. When not working, Robert enjoys travelling, especially to warmer climates during the Colorado winters. |
Gary Camp, OTR (Loveland)

Gary obtained a degree in Occupational Therapy from the University of North Dakota in 1971 following a US Army rotation in Vietnam. He has a strong interest in orthopedic injuries of the upper extremity (thumb, fingers, wrist, hand and elbow). He honed those skills while employed at the Mayo clinic for 10 years where he developed a successful out-patient hand therapy clinic. After moving to Colorado, Gary worked at PVHS, the Denver VA Medical Center, and McKee hospital where he was the Director of Rehabilitation as well as serving as an Army Reservist until retirement . Gary joined Foothills Orthopedic & Sport Therapy, P.C. in 2004 and has extensive experience in the evaluation and treatment of Upper Extremity diagnoses and is especially skilled in fabricating custom splints and braces in order to meet specific rehabilitation goals. An area resident since 1981, Gary is active in community activities, Loveland Rotary Club, playing golf and offering musical entertainment in local senior residences and healthcare facilities. |
Andrea Siener, PT, MSPT, ATC (Loveland)

Andrea joined Foothills Orthopedic & Sport Therapy, P.C. after 6 years of private practice Physical Therapy clinic ownership. Her clinical experience is primarily orthopedic rehabilitaion with a strong interest in treating knee and shoulder injuries. She has also specialized in treating patients with dizziness, vertigo and gait imbalance since becoming a certified vestibular therapist in 2004. This treatment approach is especially helpful for patients who have fallen or at risk for falling. Andrea is also a certified provider for Sole Supports custom foot orthotics. Andrea received a B.S. degree in Sports Science from Indiana University in 1991 and a M.S. degree in Physical Therapy from the University of Indianapolis in 1993. She is a certified althletic trainer (ATC). She served as the secretary for the Northeast District of the Colorado APTA for 4 years. Outside of work, Andrea enjoys spending time outdoors with her two children and husband. Her recreational interests include mountain biking, road cycling, skiing, snowboarding, camping, hiking, backpacking and rock climbing. Andrea is an active participant in Team BOB, a community service mountain biking organization for women. |
Lennea Zink, PT, MSPT (Loveland)

Lennea is a Colorado native and has been a Loveland resident since 1991. She graduated from Loveland High School where she lettered ten times while playing volleyball, basketball, and running track. Following graduation, she attended Northeastern Junior College in Sterling where she continued her volleyball career. In 1999, she graduated from Colorado State University with a B.S. in Exercise and Sport Science. She attended Regis University in 2001 where she obtained a Master's degree in Physical Therapy. Lennea was recruited by Foothills Orthopedic and Sport Therapy in 2005. Participating in athletics throughout her life has led her to develop a strong interest in out-patient orthopedics and sport therapy. She enjoys taking continuing education classes focusing on manual therapy, aquatic (pool) therapy, and women's health issues. She enjoys spending time with her husband and three children. Her free time is spent jogging, camping, cycling, playing volleyball, and volunteering at her children's schools. |
Sherie Edwards, PT, MSPT, DPT (Loveland)

Sherie Edwards, PT received her BS of Finance from Bentley College in 1989, a Master of Science of Physical Therapy and Clinical Doctorate of Physical Therapy from the Massachusetts General Hospital Institute of Health Professions (MGH IHP) in Boston in 1999 and 2003 respectively. Sherie moved to Colorado in the fall of 2008 from Boston, where she had worked extensively with the orthopedists at the Harvard affiliated Massachusetts General Hospital. Her clinical experience is primarily in orthopedics and sports therapy, with areas of specialty in treating shoulder pathology and dysfunction and post-operative clientele. Sherie also utilizes aquatic therapy (pool therapy) for clients that have difficulty with land-based activities. Sherie strongly believes in continuing education and has served as a Teacher's Assistant at the MGH IHP in Boston as well as a Clinical Instructor to Physical Therapy and Physical Therapy Assistant students and interns. She continues to attend conferences and courses to expand her knowledge and keep current on the latest techniques and research for continued evidence based practice. Sherie has been a member of the American Society of Shoulder and Elbow Therapists (ASSET) for 3 years. Sherie believes that sharing knowledge with the community is important and had created community outreach programs in ergonomics and footwear while working in Boston. She enjoys outdoor activities including hiking, kayaking, swimming, skiing and travel. |
Lee Perkins, PT, MPT (Ray PT/Fort Collins)

Lee has been practicing outpatient orthopedics since 1996. He received his Bachelor of Science in Biology from the University of California Riverside in 1994 and then completed his Masters in Physical Therapy at the University of Southern California in 1996. Lee initially worked at Chapman University Hospital, then at ProSport Physical Therapy, in Newport Beach, California where he was acting clinical director for over 7 years. During this time he also taught at Concordia University in Irvine, California, as an Associate professor of therapeutic modalities and exercise. Lee has been very involved with physical therapy education, sharing his knowledge with numerous physical therapy students over the years as well as attending annual continuing education courses to expand his own knowledge. Lee and his wife Wendi moved to Ft. Collins and he joined Ray Physical Therapy in December 2007. Outside the clinic, Lee and Wendi enjoy outdoor activities such as mountain biking, dirt biking, hiking and skiing.

Born and raised in Nebraska, Ginny and her husband and two sons have lived in Loveland since 1986. She has worked at Foothills as the Billing Manager since 2001. She enjoys bowling, biking and spending time with her family. |
Sue Helm, Front Desk (Loveland)

A native Coloradoan, Sue, her husband and two sons have lived in the Loveland area since 1995 and she has been with Foothills since 2003. She loves meeting & helping new people every day. Her interests include reading, gardening and baking. |

Debbie joined Ray Physical Therapy in 2005 and then helped with the merger between Ray and Foothills. She enjoys cycling and cooking. |
Dani Jenkins, Aide (Loveland)

Dani Jenkins began working for Foothills in June of 2008 after graduating from the University of Wyoming with a major in Kinesiology. She plans to one day further her education in Physical Therapy, but for now explores the outdoor activities that Colorado has to offer. She enjoys cycling, hiking, and spending time with her family. |
